The surgery uses tissue from other parts of the body, typically skin, fat, and blood vessels from the forearm, though tissue from the thigh or abdomen may also be used. The result is a natural-looking neophallus that allows standing urination and preserves tactile and erotic sensitivity.
The phalloplasty surgery is often combined with testicular implants and aesthetic refinement procedures to align with the trans man’s expectations. Additionally, a penile prosthesis can be implanted in a separate procedure to enable penetrative sexual intercourse. - Metoidioplasty:
Metoidioplasty, or metaioplasty, is a surgical procedure that aims to enlarge the clitoris, which has been augmented through hormone therapy, to create a neophallus. This neophallus allows standing urination and retains full tactile and erotic sensitivity. However, it does not enable penetrative sexual intercourse. - Hysterectomy:
This surgery involves the removal of internal female genital organs, such as the uterus and ovaries. This is often one of the first masculinization surgeries requested by trans men. - Testicular-Prosthesis:
Testicular prosthesis are anatomical structures resembling male testicles. They are typically placed after a phalloplasty or metoidioplasty to achieve a more complete appearance. This is a low-risk surgery that gives the scrotum a very natural look. - Mastectomy:
This surgery removes breast tissue while preserving the areola and nipple. It is often one of the first steps in the transformation from female to male.

How long does it take to recover from genital surgery for trans men?
Recovery time depends on the type of surgery and individual characteristics. For both metoidioplasty and phalloplasty, a urinary catheter is typically used for the first 21 days post-surgery. Returning to normal life generally takes a minimum of 4 to 8 weeks, provided no strenuous or physically demanding activities are undertaken.
