Aesthetic genital plastic surgery is a technique performed as part of vaginoplasty or colovaginoplasty procedures carried out at IM GENDER. However, it is also performed separately in cases where trans women who have undergone vaginoplasty elsewhere are dissatisfied with the aesthetic result and require a second procedure to improve it.
What does aesthetic genital plastic surgery involve?
Aesthetic genital plastic surgery or vaginal aesthetic surgery for trans women involves the careful design and shaping of the labia minora, clitoris and clitoral hood, vaginal introitus, perineal area, and interlabial mucosa. These aesthetic elements are essential to achieve a visually and functionally satisfactory feminine genital appearance. This configuration is constructed using scrotal skin, so the quality and quantity of tissue prior to surgery are crucial.
At IM GENDER, vaginal aesthetic surgery is routinely performed during the same procedure as the vaginoplasty to achieve a vulva that is aesthetically similar to that of a cisgender woman.

Procedures to improve the appearance of the vagina
There are different treatments to improve the appearance of the genitals. These are the most common:
Labiaplasty
It is a procedure that reduces the length of the labia minora. The labioplasty is the most demanded treatment of vaginal surgery in trans women and even cis women.
Clitoral hood reduction
A clitoral hood reduction is a surgery to decrease the size and prominence of the clitoral hood. This aesthetic procedure is often performed at the same time as labiaplasty.
Monsplasty
A monsplasty is a surgery designed to reduce bulging of the mons. The result is a mons veneris with less fat, skin and volume.
The relationship between vaginoplasty and vaginal aesthetic procedures
Aesthetic refinement of the vulva is usually carried out at the same time as the vaginoplasty, whether it’s a penile inversion, skin graft technique, or colovaginoplasty. For most patients at IM GENDER, vaginal aesthetic surgery is an integral part of the overall vaginoplasty.
Regardless the technique used, the goal is always to create a vulva and vagina that resemble those of a cisgender woman in terms of both function and appearance.
Why postpone aesthetic genital plastic surgery?
There is no medical reason to postpone vaginal aesthetic surgery during vaginoplasty. Therefore, we highly recommend performing both procedures at the same time. This way, you won’t have to undergo two recoveries, just one.
When is vaginal aesthetic surgery done separately from vaginoplasty?
As mentioned, this typically occurs when a patient visits IM GENDER because they are not satisfied with the aesthetic results achieved elsewhere.
Over time, significant weight fluctuations — whether gain or loss — can alter the appearance of the vulva and vagina, just as they might in cis women. These changes may lead to a desire or need for aesthetic revision.
In rare cases, natural scar formation can result in minor asymmetry of the labia, which may require a small touch-up. However, this is not considered a new aesthetic surgery, but rather a minor correction.
Constant innovation and new techniques
Over the years, IM GENDER has developed and adopted new surgical techniques that offer even better results. This means that trans women who had vaginoplasty in the past may now be candidates for enhancing their vulval aesthetics through these newer, more advanced procedures.
