MTF top surgery
Although hormone therapy significantly helps breast growth, in many cases it is not enough or does not turn out to be the one desired by transgender women. In these cases, a MTF top surgery is recommended, also called augmentation mammoplasty.

POSTOPERATIVE PERIOD
Your surgeon will usually provide you with guidelines for postoperative care. Among these: when to start wearing a bra after surgery, how to sleep or when is the right moment to have a shower after the surgery.
- In general, you may be advised to wear a supportive, non-underwire bra or a surgical compression garment for several weeks to provide support and help reduce swelling.
- You should sleep on your back for at least the first month after your chest surgery. Although it will always depend on the recovery of each patient.
- You will usually be able to shower within 24-48 hours after surgery, but it’s essential to follow your surgeon’s instructions. Be cautious not to let the water pressure hit your incisions directly and avoid submerging your breasts in water (e.g., baths, swimming pools) until your incisions are fully healed.
MTF TOP SURGERY RECOVERY
Recovery time can vary, but most patients can expect to return to work and light activities within 1-2 weeks. It is important to avoid heavy lifting and strenuous exercise for 4-6 weeks to allow the body to heal properly.
Once the drains are removed, the patient’s recovery is much faster. After 10 days of breast augmentation surgery, the decrease in inflammation begins to be noticed. In a month, when the bands and the bra are removed, the results of the operation begin to be appreciated. Although it will not be until 6 months when the breast can be seen 100% recovered.
The result of MTF top surgery is an increase in breast size and a more feminine contour to the chest. The exact result will depend on the patient’s anatomy, implant type and size, and surgical technique used. Most patients report feeling more confident and comfortable in their bodies after the procedure. It is important to have realistic expectations and to communicate your desired outcome with your surgeon during the consultation process.
RISKS
All surgeries always have some risks. These are rare complications, but they can happen:

- Hematomas and seromas under the skin.
- Infection that may require treatment with oral antibiotics.
- Loss of nipple sensation.
- Irregular contours.
- Capsular contracture.
- Anaesthesia reaction.
FAQS ABOUT MTF TOP SURGERY
What is MTF top surgery?
MTF (male to female) top surgery is a surgical procedure performed on transgender women as part of their gender affirmation process. The surgery involves the placement of breast implants, typically made of silicone or saline, to enhance the size and shape of the breasts. The goal of MTF breast augmentation is to provide transgender women with more feminine-looking breasts that are in proportion to their body, helping them align their physical appearance with their gender identity and improve their overall quality of life and well-being.
What is transgender breast augmentation like?
Breast augmentation is a relatively short procedure, usually lasting between one and two hours. General anaesthesia is typically used.
There are three possible ways in which the prosthesis can be implanted:
- Axillary incision: the prosthesis is inserted via an incision in the armpit, which strategically hides the scar.
- Inframammary fold incision: the incision is made in the inframammary fold and hidden in the groove under the breast.
- Areolar incision: it could be only in the inferior area or all around the areola.
Your plastic surgeons will evaluate all the possibilities according to your characteristics and medical history before advising you on one of these options.
It is common for drains to be inserted during the operation, which are removed after approximately 48 hours. They help to reduce inflammation and post-operative discomfort to a minimum.
What type of implants are used in MTF top surgery?
There are several types of implants that can be used in MTF top surgery. The most commonly used implants are silicone and saline. Silicone implants are filled with a cohesive silicone gel that closely mimics the feel of natural breast tissue, while saline implants are filled with sterile saltwater solution. Your surgeon will help you determine which type of implant is best for you based on your individual goals and anatomy. They will also discuss the different shapes, sizes, and textures of implants available to help you achieve your desired outcome.
Will I lose sensation in my breasts or nipples after MTF breast augmentation?
There is a possibility of temporary or permanent changes in nipple or breast sensation following the surgery. However, many patients experience a return of sensation as the nerves heal over time.
What is the price of a MTF top surgery?
The price of a trans breast operation can be different depending on the case of the person. It must also be taken into account if the chest surgery is decided to be combined with other treatments. A first visit is recommended to assess the patient’s case to make a personalised budget.
