It is important not to self-medicate or use alternative treatments without consulting your doctor and the IM GENDER team.

I cannot locate the vaginal opening properly. Why?

It is common for some anatomical confusion to arise during the first few washings or dilations. You may be trying to insert the cannula or dilator into the urethra, which can cause pain. The vaginal opening is wider and located just below. Using a hand mirror and good lighting will help you get to know yourself better and gain confidence. Over time, you will become familiar with your new anatomy.

What is this mucus I am expelling?

During the first month after surgery, it is very common to observe mucus secretions, with traces of blood or yellowish and/or greenish tones. Do not be alarmed, this is normal. Especially when a colovaginoplasty has been performed. What is happening is that the colon cells continue to produce mucus. It is also normal to see traces of antiseptic (brownish in colour) or residual blood from the surgery. If the discharge has a strong odour or causes discomfort, you can increase the frequency of saline washes.

I feel intense sensitivity in the clitoral area

Don’t be alarmed, these types of sensations are signs that the sensory nerves are in the process of reconnecting. When this happens, it is not unusual to experience tingling, increased sensitivity or even occasional pain. Remember that it is important not to force genital self-exploration or seek pleasurable sensations during the first three months. Patience and respect for your own body are key to a good recovery.

Can I shower? What soap should I use?

Yes, not only can you shower, but you should shower once you have left the hospital. During vaginoplasty recovery, it is essential to keep the area clean, as this promotes healing and prevents infections. Use neutral or glycerine soap, never perfumed, and make sure you dry the area with a hairdryer on a cool setting (never hot). Do not rub with a towel or cloth, no matter how soft it may seem. Do not apply creams or healing products without medical advice.

When can I resume physical exercise?

Provided that your recovery is progressing well, you can resume gentle physical activity after the second month. This means going for walks or doing stretching and controlled movements. However, you should avoid abdominal exercises, jumping or high-impact activities until the IM GENDER team advises you otherwise.

My labia aren’t symmetrical. Is that serious?

No, it’s part of the healing process. During the first few weeks after surgery, it’s common for the labia majora to show some swelling or slight asymmetry. Over time, everything will settle into its final shape.

I can’t find my clitoris. Is that normal?

Yes. The clitoris is partially covered by its hood, and post-surgical swelling can make it harder to see. Don’t worry—it hasn’t disappeared. As the swelling decreases over the following weeks, you’ll be able to identify it more clearly.

My dilator fell or isn’t going in like before

If the dilator falls on the floor, it can break or lose effectiveness. In that case, contact the IM GENDER patient care team to get a replacement. Never substitute it with a sex toy unless explicitly recommended by the IM GENDER team.

If you feel the dilator isn’t going in as easily as before, don’t panic: swelling, tension, or fatigue can make dilation more difficult. Try to relax, use water-based lubricant, and take your time. If the difficulty continues, request a follow-up appointment.

During vaginoplasty recovery, can I use scar creams or intimate products?

We don’t recommend using these types of creams during the first month of vaginoplasty recovery. The area needs to heal naturally. The best approach is basic hygiene, which will be explained to you at hospital discharge. After the first check-up, the medical team will assess whether you can begin using regenerative creams or complementary treatments. But never start any such treatment without consulting your doctor.

What if I see traces of blood after dilation?

The vaginal mucosa is sensitive, especially at the beginning. It’s very common to see thin streaks of blood or reddish residue on the gauze or dilator. As long as there isn’t heavy bleeding or intense pain, there’s no reason to worry.

When can I have sex again?

At IM GENDER, we usually recommend waiting at least three months before having penetrative sex or intense erotic exploration after vaginoplasty. This is a crucial healing stage that must be respected to ensure good outcomes. When the time comes, use water-based lubricants and communicate openly with your partner. When can I swim in the sea or a pool?

You will need to wait around three months. Swimming in pools or natural waters is not recommended before the fourth month of recovery, even if you think you are fine. It is essential to wait until all wounds are completely closed and healed.

How do I know if something is wrong?

Trust your intuition, but avoid getting alarmed over every little detail. Consult your doctor if you have a persistent fever, heavy bleeding, pain that does not improve with painkillers, a strong odour or abnormal secretions after the first month.
